Note: The use of this form is considered to be an official entry. Per CFA show rules, your e-mail address on this form is considered to be your signature. Entries MUST be paid in full to the sponsoring club once this form is submitted - within 10 days of receipt, or prior to the start of the show, whichever is first.

Starting with the 2023-24 show season, show rule 6.36 specifies that all shows in Regions 1-7, excluding Hawaii, must close to entries no later than 11:59 pm (Central Office time) on the Monday before the opening of the show. Shows may elect to close to entries prior to that time.
